We care for individuals and the community
Every year, thousands of people benefit from our community focused approach to care. That makes a huge difference to patients and their families, as well as the broader WA community – Home Hospital alleviates the strain on our finite public health and hospital resources.
- We treat over 650 people with acute and sub-acute conditions in Perth every day. That equates to 12,185 patient episodes in the last financial year.
- On average, we care for 60-70 patients per day who would otherwise be hospitalised for conditions that can be managed with 24-hour medical governance at home.
- 300 patients are discharged into Silver Chain Home Hospital’s Post-acute Care service every month. That’s roughly 3,500 people every year who are discharged earlier from surgical wards, thereby freeing up local hospital inpatient capacity.
- We receive more than 250 referrals from general practitioners (GPs) and medical specialists for sub-acute conditions every month. Without our sub-acute in-home care services, it’s likely that these patients would require ongoing treatment in a public inpatient or outpatient facility.
- More than 2,200 people were assessed by our Priority Response Assessment (PRA) team in their home or residential aged care facility. This reduces the demand on our community’s ambulance service and the emergency departments in local hospitals.
